Behind the Theatre Doors: Leadership in Surgical Services

by: Christian Valenzuela

Healthcare leadership is often misunderstood. Many imagine it as boardroom meetings and spreadsheets. In reality, effective clinical leadership sits right at the intersection of strategy, patient safety, and frontline support.

This article shares insights from an Operating Theatre Manager at Royal Cornwall Hospitals NHS Trust, exploring the role, career journey, and the deeper fulfilment that comes from leading teams while improving patient care.

My Roles and Responsibilities

I am currently an Operating Theatre Manager at Royal Cornwall Hospitals NHS Trust, with experience in Clinical Site Management and previous experience in Nursing Education, Emergency Department and Delivery Suites.

My current role centres around operational leadership, patient safety, workforce planning, governance, and service delivery.

A typical day?

Strategic at sunrise, operational by mid-morning, problem-solving all afternoon.

I oversee elective and emergency theatre flow, manage staffing and escalation, respond to incidents, support consultants and frontline teams, and ensure compliance and quality standards are maintained.

Because of my background in Clinical Site Management, I don’t just see theatres — I see the entire hospital ecosystem. I understand how bed capacity, emergency pressures, and critical care flow impact surgical services. That wider lens helps me make balanced, system-aware decisions.

Some days I manage performance targets.

Other days I manage emotions.

Both matter equally.

Patients and Cases

As a Theatre Manager, I oversee a broad spectrum — trauma, emergency, elective, and complex surgical cases.

I may not be managing one individual patient at a time, but I am responsible for the system that safely supports hundreds of patients each month.

My focus is ensuring that care is delivered efficiently, safely, and with the right workforce in place.

Contribution to the Wider Healthcare Team

My role bridges strategy and frontline care.

I advocate for theatre teams at senior meetings, translate corporate objectives into operational reality, and ensure executive decisions align with what is safe and practical on the ground.

With my Clinical Site Management experience, collaboration comes naturally — with matrons, bed managers, ED, critical care, estates, and corporate teams.

Theatres do not operate in isolation. We are part of a larger, interdependent system.

Career Path and Application

Inspiration behind the Specialty

I’ve always been drawn to high-performing teams and environments where precision and leadership matter.

Theatres are intense, dynamic, and require calm leadership under pressure — that challenged and inspired me.

As an international nurse, I was also motivated by representation. I wanted to demonstrate that leadership roles in the NHS are accessible to those willing to work hard, remain consistent, and step forward with courage.

Growth for me has always been intentional.

Transitioning into the Role

It was a combination of courage to step up, clinical progression, leadership exposure, mentorship, and formal application.

I took on responsibilities early, volunteered for improvement projects, engaged in governance work, and gained valuable experience in Clinical Site Management — which broadened my operational perspective.

I did not wait for opportunities to arrive; I positioned myself to be ready when they did.

Qualifications and Experience

Leadership development programmes, quality improvement involvement, incident management experience, workforce planning exposure, and hospital-wide operational management all contributed.

But beyond qualifications, credibility was key.

When teams trust your judgment clinically and operationally, leadership becomes influence — not just a title.

Joys & Fulfilment

What Makes the Role Meaningful

Seeing systems improve and people grow.

There’s deep satisfaction in watching a pressured theatre list run smoothly because the team is aligned.

But even more fulfilling is seeing a junior nurse develop confidence, step into leadership, and realise their potential.

Leadership, for me, is about enabling others.

A Meaningful Moment beyond the Hospital

Beyond hospital walls, one of my proudest moments is serving as Chairperson of the Filipino Community in Cornwall (FCCO).

I chair volunteer-led initiatives such as Pyesta in Cornwall and Filipino Day in Cornwall — events that celebrate multi-culture, unity, and community.

Through our team’s dedication and fundraising activities, we were able to save enough funds to support four scholars with their education in the Philippines.

That achievement means more than applause or recognition. It represents collective effort, shared purpose, and tangible impact that everyone had poured into.

I love my team — we are all different personalities, different strengths, and different opinions — but somehow we maintain a homeostatic balance. There’s healthy debate, lots of laughter, occasional stress… but always alignment with our values.

Leadership in healthcare is not simply about authority — it is about responsibility, influence, and service. From managing theatre operations to empowering colleagues and supporting communities beyond the hospital, the role of an Operating Theatre Manager reflects how clinical leadership can shape both healthcare systems and lives.

Wow, it’s impressive to read about how an operating theatre manager handles everything from the early morning to make sure surgeries run smoothly. It really shows how important planning, teamwork, and quick thinking are in a busy healthcare setting. I can imagine how challenging it must be to stay organized while keeping everyone on the same page and supporting the team through long shifts. Reading this also made me think about how much planning and support students need when juggling multiple deadlines. That’s why many people look for Reliable academic writing help in UK to stay on top of assignments without getting overwhelmed. It’s interesting to see how proper guidance can make tough situations feel a lot more manageable.
